Certified divers disclaimers
1. Isrotel is not responsible for the loss and/or damage of personal equipment and/or personal dive gear that was left unattended in the club.
2. I hereby confirm that I checked all of the items that I rented and found them in order.
3. I hereby confirm that I will return all the equipment I rented in a working condition and to be responsible for any damage or loss of equipment.
4. I know that if I will not return the equipment on time, I will be charged for an extra rental day.
5. I herbey confirm that I will dive according to safety procedures, and the diving law in Israel.
6. The equipment is for certified divers who have valid insurance only, and is not transferable. I know that giving diving equipment to non certified divers is a felon.
7. I hereby confirm that I know the medical limitations for recreational diving and that I am healthy and do not have any medical issues that can prevent diving.
8. I hereby confirm that I will dive in the limits of my certification and according to the diving law in this country.
For your information, this form is valid for the dates of your visit only, the club is not keeping copies of your documents for a long time and will not be able to provide them to you if asked.
Take note! Filling out this form does not mean that we will reserve a spot for your dives, nor that we will reserve diving equipment for you. Gear rental or participation in diving activities is subject to availability only. It is recommended to reserve a spot in refresher or guided dives via the reservation office.
